Postpartum Weight Loss: Health Concerns After Pregnancy - Internal Medicine

Significant weight loss after childbirth?

Hello Doctor, before my pregnancy, my weight was approximately 65 kg (height 159 cm, BMI was overweight).
During my pregnancy, I had gestational diabetes and used short-acting insulin (once in the morning).
At the time of delivery, I weighed about 71 kg, and now, 15 months postpartum, my weight has significantly decreased to 53 kg.
I am still breastfeeding.
My eating schedule is irregular; sometimes I get very hungry while caring for the baby and don’t have time to eat, especially in the evening when I often feel thirsty.
I drink a lot of water and frequently go to the bathroom, and I wake up at night feeling very hungry.
Currently, I am overseas and want to understand my health status before getting the COVID-19 vaccine, as I have lost around 15 kg.
I visited a local family medicine clinic for a check-up, and the doctor reviewed my blood test results, stating that I do not have diabetes, but I have iron deficiency anemia.
There was no explanation for my weight issues, and due to language barriers, a friend suggested I consult with E Hospital in Taiwan.
Should I undergo further examinations? The highlighted parts of my blood test report are in German (translated using Google Translate):
Iron 33 ug/dl
Transferrin saturation 6%
Ferritin 7 ng/ml
Vitamin D (25-OH) total 13 ng/ml
Vitamin D (25-OH) total IFCC 31 nmol/l
Hemoglobin 10.8 g/dl
Hematocrit (Volume fraction) 0.37 l/l
MCV (mean cell volume) 78
MCH (mean cell Hb) 23
MCHC (mean cell Hb concentration) 29 g/dl
RDW-CV 18%
Transferrin 3.49 g/l
The report does not highlight the following:
Hemoglobin A1c 5.6%
Hemoglobin A1c IFCC 37.0 mmol/mol
Total protein 78 g/l
Symptoms I had before pregnancy (which I still have):
*Iron deficiency anemia due to heavy menstrual flow (with uterine fibroids growing on the endometrium; I had a cervical conization surgery before pregnancy, removing about 30% of the fibroids)*
*Numbness from the feet to the calves (feels like there’s a layer of cloth when touched)*
*Recurrent itchy red rash on the neck*
*Sweaty vesicular dermatitis on the hands*
*Taking iron supplements causes severe itching (once, intravenous iron caused immediate hives)*
*Blood pressure is generally low, averaging around 88/46 (I believe I have a low blood pressure constitution; my mother also has low blood pressure)*
*Fasting blood sugar in the morning measures between 98-101.*
Thank you, Doctor! :)

Postpartum weight loss can also be evaluated by checking thyroid function, as thyroid dysfunction can affect weight.

More Info

Postpartum weight loss and the associated health concerns can be quite complex, especially for individuals who have experienced complications during pregnancy, such as gestational diabetes. In your case, you have provided a detailed account of your weight changes, dietary habits, and various health issues, including iron deficiency anemia and low blood pressure. Let’s break down these aspects to better understand your situation.

Weight Changes Postpartum
You mentioned that your weight decreased significantly from 71 kg at the time of delivery to 53 kg 15 months postpartum. This is a substantial loss of 18 kg, which may be concerning, especially considering your initial BMI categorized you as overweight. While some women may experience weight loss after childbirth, especially if they are breastfeeding, such a drastic change can sometimes indicate underlying health issues.

Nutritional Considerations
Your irregular eating patterns, particularly during breastfeeding, can contribute to weight loss. Breastfeeding increases caloric needs, and if you are not consuming enough calories, your body may start to utilize stored fat and muscle for energy. However, it’s crucial to ensure that you are still getting a balanced diet rich in essential nutrients, especially since you are experiencing symptoms of iron deficiency anemia.
Iron deficiency can lead to fatigue, weakness, and other health complications, which may further complicate your postpartum recovery. The low ferritin levels (7 ng/ml) and transferrin saturation (6%) you reported indicate a significant deficiency, which needs to be addressed. It’s advisable to consult with a healthcare provider or a nutritionist to develop a meal plan that includes iron-rich foods (like lean meats, beans, lentils, and fortified cereals) and vitamin C sources to enhance iron absorption.

Health Monitoring
Given your history of gestational diabetes, it’s essential to monitor your blood sugar levels regularly, especially since you mentioned fasting blood glucose levels between 98-101 mg/dL. While these levels are not indicative of diabetes, they are on the higher end of normal, which warrants attention. Maintaining a balanced diet and regular physical activity can help manage blood sugar levels effectively.

Symptoms and Further Evaluation
The symptoms you described, such as numbness in your legs, itchy rashes, and low blood pressure, should not be overlooked. These could be related to your anemia or other underlying conditions. The numbness may suggest nerve involvement, possibly due to nutritional deficiencies or other factors. The itchy rashes could be a reaction to certain foods or medications, and it’s important to identify any triggers.

Since you are experiencing multiple health concerns, it would be prudent to seek further evaluation. A comprehensive assessment by a healthcare professional, including blood tests to check for vitamin deficiencies (like vitamin D, which is also low in your results), thyroid function, and possibly a referral to a specialist for your skin and nerve symptoms, would be beneficial.

Conclusion
In summary, postpartum weight loss can be influenced by various factors, including dietary habits, breastfeeding, and underlying health conditions. It’s crucial to ensure that you are consuming a balanced diet to support your health and recovery. Regular monitoring of your blood sugar levels and addressing your anemia is essential. Given your complex health history, further evaluation by healthcare professionals is recommended to ensure that all aspects of your health are being addressed appropriately.
If you are considering vaccination against COVID-19, it’s advisable to discuss your current health status with your healthcare provider to ensure that you are in the best possible condition for vaccination.
