Measurement errors in leg length discrepancy and inability to stand for prolonged periods?
Hello Doctor: I would like to ask, during my first visit to Hospital 804 for an X-ray, the measurement of my right leg was 2.15 cm longer than my left leg, but during my second visit to Veterans General Hospital, the measurement changed to 1.5 cm.
Is such a significant difference in values normal? Also, what does X-ray scanometry mean? I have difficulty standing for long periods due to my leg length discrepancy, and I experience lower back pain.
Additionally, there is a difference in the size and strength of my legs.
Can this be resolved through exercise? If so, what kind of exercises should I do? Thank you, Doctor, for taking the time to respond!

Dr. Zhan Xian reply Orthopedics
Hello, scanometry of the lower extremities is primarily used to assess leg length and deformities.
There may be some variations in measurements due to different hospitals, software, and physicians.
If your leg length discrepancy is between 1-2 centimeters, you can initially use shoe inserts for assistance.
Strength training is also necessary.
If you still have concerns, please visit an orthopedic clinic for a physical examination and X-ray interpretation.

More Info
Hello! Thank you for your question regarding leg length discrepancy (LLD) and the associated issues you are experiencing. It’s understandable to be concerned about the differences in measurements you received from two different hospitals. Variability in measurements can occur due to several factors, including the positioning of the patient during the X-ray, the technique used by the radiologist, and even the equipment itself.
Measurement Variability
In your case, the difference between 2.15 cm and 1.5 cm is not uncommon. Factors such as the angle of the X-ray, the alignment of the legs, and even the specific method used to measure the lengths can lead to discrepancies. The term "scanometry" refers to a method of measuring the lengths of the lower extremities using imaging techniques, which can help in assessing leg length discrepancies more accurately. However, it is essential to have a consistent method of measurement to ensure reliability.
Addressing Pain and Discomfort
Regarding your pain and discomfort, it is not unusual for individuals with LLD to experience issues such as lower back pain and muscle imbalances. The difference in leg lengths can lead to an uneven distribution of weight and altered biomechanics, which can contribute to discomfort in the lower back and legs.
Exercise and Rehabilitation
The good news is that certain exercises can help alleviate some of these issues. Strengthening and stretching exercises can improve muscle balance and support your joints. Here are some recommendations:
1. Strengthening Exercises:
- Single-leg Squats: This exercise helps strengthen the muscles around the knee and hip on both sides, promoting balance.
- Step-ups: Use a step or a low platform to step up and down, focusing on using the leg that is shorter to build strength.
- Bridges: Lying on your back with your knees bent, lift your hips off the ground to strengthen the glutes and lower back.
2. Stretching Exercises:
- Hamstring Stretch: Stretching the hamstrings can help relieve tension in the lower back.
- Hip Flexor Stretch: This can help alleviate tightness in the hips, which may be contributing to your discomfort.
- Quadriceps Stretch: Stretching the front of the thigh can also help balance muscle tension.
3. Core Strengthening:
- Planks: Strengthening your core can provide better support for your spine and help alleviate back pain.
- Bird-Dog: This exercise promotes stability and coordination between the upper and lower body.
Orthotic Solutions
In addition to exercises, using orthotic shoe inserts can help compensate for leg length discrepancies. Custom-made orthotics can provide additional support and help align your body better, potentially reducing pain and discomfort.
When to Seek Further Help
If your pain persists or worsens despite these interventions, it would be wise to consult with an orthopedic specialist or a physical therapist. They can perform a thorough assessment and may recommend imaging studies or other interventions tailored to your specific needs.
In summary, while the variability in your leg length measurements is not unusual, addressing the associated pain and discomfort through targeted exercises and possibly orthotic solutions can be beneficial. Always consult with a healthcare professional before starting any new exercise program, especially if you have existing pain or medical conditions. Thank you for your question, and I wish you the best on your journey to improved health!
