Shoulder Pain: Exploring Treatment Options and Rehabilitation Strategies - Rehabilitation

Shoulder pain

Hello Doctor: Since July of last year, I have been experiencing pain in my left shoulder that extends to the entire scapula.
An ultrasound in August revealed a shoulder tear, and after undergoing electrical therapy and microwave physical therapy, I improved by 90%.
In October, the pain spread to my entire arm down to the wrist, affecting my sleep.
An MRI showed fluid accumulation, a tear, aging, calcification, and adhesion issues.
I have been taking anti-inflammatory and pain relief medication for over two months, along with IMS treatment.
I stopped treatment at the end of January and have not needed pain medication since.
The range of motion in my arm has improved significantly, and I currently feel about 70% recovered, though I still experience pain and some movements are still limited.
Since I have passed the stage of unbearable pain, I would like to know if I should consider undergoing dilation therapy, or if I can simply focus on stretching and strengthening my muscle endurance.
I am concerned that injecting saline for dilation without ultrasound guidance may lead to inaccurate injection points, potentially resulting in worse outcomes.
Thank you!

Dr. Lin Ruixiang reply Rehabilitation

Ms.
Luo: Hello, your condition shows an increase in joint angle and a decrease in pain, which indicates that you are in the resolution phase of frozen shoulder.
The typical duration of this condition is about six months.
The primary benefit of shoulder joint distension is that it can reduce the duration of the condition by approximately 70%.
While injections into the shoulder joint do not necessarily require ultrasound guidance, it is preferable to use ultrasound-guided injections in the current medical environment.
The side effects of distension, as you mentioned regarding the injection site, include another serious complication, which is joint capsule rupture.
However, since saline is used for the injection, it does not have any adverse effects on the body.
Nonetheless, pain caused by technical errors can still impact the course of the condition.

Shoulder pain can be a complex issue, especially when it involves injuries such as rotator cuff tears, bursitis, or other degenerative changes. Based on your description, it seems you have been through a significant journey with your left shoulder pain, which started in July of last year and has involved various treatments, including physical therapy, medication, and imaging studies.

From your ultrasound and MRI findings, it appears that you have a combination of a rotator cuff tear, fluid accumulation (which may indicate bursitis or inflammation), and possibly calcific tendinitis. These conditions can lead to pain that radiates down the arm, affecting your daily activities and sleep quality. It's encouraging to hear that you have seen some improvement with physical therapy and that you are no longer relying on pain medication. However, the persistence of pain and limited range of motion suggests that further intervention may be necessary.

In terms of treatment options, there are several avenues you can explore:
1. Continued Rehabilitation: Since you have already experienced significant improvement with physical therapy, it may be beneficial to continue with a structured rehabilitation program. This can include targeted exercises to improve strength and flexibility in the shoulder, as well as modalities like ultrasound or electrical stimulation to reduce pain and inflammation. A physical therapist can tailor a program specifically for your needs, focusing on both stretching and strengthening the shoulder muscles.

2. Injections: Corticosteroid injections can provide significant relief from inflammation and pain, especially if there is bursitis or significant inflammation present. However, as you mentioned, the concern about accuracy in injection without imaging guidance is valid. If you consider this option, it would be best to have the injection performed under ultrasound guidance to ensure proper placement and maximize the chances of success.

3. Surgical Options: If conservative treatments fail to provide adequate relief and your functional limitations persist, you might consider discussing surgical options with your orthopedic surgeon. Procedures such as arthroscopic debridement, rotator cuff repair, or subacromial decompression can be considered depending on the specific findings and your overall health status. The decision for surgery should be made collaboratively with your surgeon, weighing the potential benefits against the risks and your personal goals for recovery.

4. Hydrodilatation: This procedure involves injecting a saline solution into the shoulder joint to help stretch the capsule and improve range of motion. It can be effective for adhesive capsulitis (frozen shoulder) but should be performed by a skilled practitioner to ensure proper technique and minimize risks.

5. Self-Management: Continuing with self-directed stretching and strengthening exercises can be beneficial. Focus on gentle range-of-motion exercises and gradually increase the intensity as tolerated. It’s essential to listen to your body and avoid any movements that exacerbate your pain.

In conclusion, it seems you are on the right track with your recovery. Continuing with physical therapy, considering injections if necessary, and discussing surgical options with your orthopedic specialist are all valid paths forward. It’s crucial to maintain open communication with your healthcare providers to ensure that your treatment plan is tailored to your specific needs and goals. Remember that recovery from shoulder injuries can take time, and patience is key.
