Uneven Breast Size and Swelling: Possible Causes - Breast and Thyroid

Significant asymmetry in breast size and swelling?

Hello, doctor.
Over the past six months, I have noticed a significant difference in the size of my breasts.
The left side is larger and firmer, while the right side feels soft and smaller, almost like fatty tissue.
Additionally, during this six-month period, I have experienced breast tenderness that has shifted from being bilateral to only the left side.
I would like to ask, is it normal for breasts to continue growing after the developmental stage? Since my left breast has only increased in size during these past six months, I am concerned that there may be an underlying issue.
Furthermore, during self-examination, I did not find any lumps, but the texture of the two sides is markedly different.
Could you please advise what might be causing this situation?

Wang, 20~29 year old female. Ask Date: 2022/04/12

Dr. Yuan Tianmin reply Breast and Thyroid

Currently, palpation is not very effective for breast examination; it is primarily recommended to use ultrasound.
It is advisable to consult a specialized clinic for a breast ultrasound.

Uneven breast size and swelling can be concerning, especially when changes occur suddenly or are accompanied by other symptoms. In your case, the noticeable difference in size and texture between your breasts, particularly with the left breast being larger and firmer while the right is softer and smaller, warrants further exploration.

Firstly, it is important to understand that breast tissue is composed of glandular tissue, fat, and connective tissue, and the proportions of these components can vary significantly between individuals and even between the two breasts of the same individual. Hormonal fluctuations, particularly those related to the menstrual cycle, can cause temporary changes in breast size and texture. For instance, during the luteal phase of the menstrual cycle, increased levels of estrogen and progesterone can lead to breast swelling and tenderness, which might explain the changes you’ve noticed.

However, since you mentioned that the left breast has become noticeably larger over the past six months, it is essential to consider other potential causes. Some possibilities include:
1. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) or other endocrine disorders can lead to uneven breast development. Hormonal fluctuations can also be influenced by stress, diet, and lifestyle factors.

2. Fibrocystic Changes: This is a common condition characterized by lumpy or rope-like breast tissue, which can lead to differences in breast size and texture. It is often influenced by hormonal changes and can cause discomfort or swelling.

3. Breast Cysts or Fibroadenomas: These benign growths can develop in breast tissue and may cause changes in size or texture. While you mentioned not feeling any lumps, it is still possible for small cysts or fibroadenomas to exist without being palpable.

4. Infection or Inflammation: Conditions such as mastitis or other inflammatory processes can lead to localized swelling and changes in breast size. This is usually accompanied by other symptoms such as redness, warmth, or pain.

5. Breast Cancer: Although less common, it is crucial to rule out breast cancer, especially if there are significant changes in size, shape, or texture. While you have not felt any lumps, breast cancer can sometimes present with changes in breast appearance or texture without a palpable mass.

Regarding your question about breast growth after the developmental phase, it is generally true that breast development stabilizes after puberty. However, breasts can continue to change due to hormonal influences throughout a woman’s life, particularly during menstrual cycles, pregnancy, and menopause.
Given the changes you are experiencing, it is advisable to consult a healthcare professional, preferably a breast specialist or a gynecologist. They may recommend imaging studies such as a mammogram or ultrasound to assess the breast tissue further and rule out any underlying conditions. Additionally, they can evaluate your hormonal levels and overall health to determine if there are any contributing factors to the changes you are observing.

In summary, while hormonal fluctuations are a common cause of breast size differences, the significant change in your left breast over the past six months should not be overlooked. Seeking professional medical advice will provide you with clarity and peace of mind regarding your breast health.
