Questions about unusual sensations during exercise?

Hello Doctor, yesterday while doing push-ups, I suddenly experienced a twitching sensation on the outer side of my right arm, similar to an electric shock, during each downward motion.
I felt something was wrong and soon stopped exercising.
After that, I didn't have any particular sensations.
Could you please tell me if it's possible that I injured something? Do I need to seek medical attention? Thank you, Doctor.

JAM, 20~29 year old female. Ask Date: 2024/03/18

Dr. Wang Zhaomin reply Rehabilitation

Muscle strains or nerve irritation can both trigger similar symptoms.
If discomfort persists, it is advisable to seek medical attention promptly for an accurate diagnosis and appropriate treatment.

Experiencing unusual sensations during exercise, such as the electric shock-like twitching you described in your right arm while doing push-ups, can be concerning. While it’s good that you stopped exercising when you felt something was off, it’s important to understand the potential causes and whether you should seek medical attention.

The sensation you experienced could be attributed to several factors. One possibility is nerve irritation or compression. The brachial plexus, a network of nerves that innervates the arm, can be affected by various factors, including poor posture, repetitive movements, or even muscle tightness. If you were performing push-ups with improper form, it could lead to strain on the shoulder or neck, which might irritate the nerves supplying the arm.

Another possibility is muscle fatigue or overexertion. If your muscles are not accustomed to the intensity or volume of the exercise, they may respond with spasms or twitching. This is particularly true if you have recently increased your workout intensity or changed your routine.

Additionally, electrolyte imbalances can lead to muscle cramps or spasms. If you have not been adequately hydrating or if your diet lacks essential minerals like potassium, calcium, or magnesium, this could contribute to the sensations you experienced.

In terms of whether you should see a doctor, it depends on a few factors:
1. Duration and Frequency: If the sensation was a one-time occurrence and has not returned, it may not be a cause for concern. However, if it recurs or is accompanied by other symptoms such as weakness, numbness, or pain, it would be wise to consult a healthcare professional.

2. Severity of Symptoms: If you experience significant pain, swelling, or if the sensation spreads to other areas, you should seek medical attention promptly.

3. Impact on Daily Activities: If the sensation interferes with your ability to perform daily activities or exercise, it’s advisable to get evaluated.

4. Underlying Health Conditions: If you have a history of neurological issues, cardiovascular problems, or other health concerns, it would be prudent to discuss these symptoms with a healthcare provider.

In summary, while the sensation you experienced during push-ups could be benign, it’s essential to monitor your symptoms. If they persist or worsen, or if you develop new symptoms, seeking medical advice is the best course of action. A healthcare professional can perform a thorough evaluation, which may include a physical examination and possibly imaging or nerve conduction studies, to determine the underlying cause and recommend appropriate treatment. In the meantime, ensure you maintain proper form during exercises, stay hydrated, and consider incorporating a balanced diet rich in essential nutrients to support muscle function.
