Shoulder pain
My mom said that after she recently carried a shopping bag on her right shoulder for about 3 minutes at the market, she started experiencing shoulder pain.
Additionally, she noticed small bruises appearing on her shoulder for several consecutive days, which she had never experienced before.
She is quite worried about this.
Could you please explain why this might be happening? Thank you!

Dr. Chen Xianchang reply Rehabilitation
Hello, thank you for your inquiry.
Here are my responses and recommendations:
1.
When lifting heavy objects with the shoulder, the muscles primarily involved are the levator scapulae and trapezius, which may become overused.
During the acute phase, apply ice for the first day or two, followed by heat therapy.
If symptoms do not improve, consider rehabilitation, deep tissue treatment, and electrical therapy.
2.
For bruising, it is important to consider the possibility of internal bleeding.
First, check for abnormalities in platelet count and coagulation factors, as lifting heavy objects may not necessarily result in bleeding.
Wishing you safety and happiness!

Shoulder pain and bruising after light activity, such as carrying grocery bags, can be concerning, especially if it is a new symptom for someone. In your mother’s case, there are several potential explanations for her shoulder pain and the appearance of bruising.
Firstly, the shoulder is a complex joint that relies on a combination of muscles, tendons, and ligaments to function properly. When carrying a weight, even for a short duration, it can place stress on these structures. If your mother has not engaged in similar activities recently, her muscles and tendons may not be accustomed to the exertion, leading to strain or minor injuries. This is particularly true if she has any underlying conditions such as rotator cuff issues, tendonitis, or arthritis, which can make the shoulder more susceptible to pain and injury.
The bruising she is experiencing could be a result of microtrauma to the blood vessels in the shoulder area. When the muscles or tendons are strained, small blood vessels can break, leading to bruising. This is more likely to occur if she has fragile skin or blood vessels, which can be a common issue as people age. Additionally, certain medications or health conditions can affect blood clotting and vessel integrity, making bruising more likely.
Another important consideration is the possibility of a more serious underlying condition. Conditions such as a rotator cuff tear, bursitis, or even a fracture can cause pain and bruising. If your mother is experiencing significant pain, limited range of motion, or if the bruising worsens or does not improve, it would be advisable for her to seek medical attention. A healthcare provider can perform a physical examination and may recommend imaging studies, such as an X-ray or MRI, to assess for any structural damage.
In the meantime, there are several strategies your mother can employ to manage her shoulder pain. Resting the affected shoulder and avoiding activities that exacerbate the pain is crucial. Applying ice to the area can help reduce swelling and alleviate pain. Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may also provide relief. Gentle stretching and strengthening exercises, once the acute pain subsides, can help restore function and prevent future injuries.
It’s also important to consider lifestyle factors that may contribute to shoulder pain. If your mother has a sedentary lifestyle or poor posture, these can lead to muscle imbalances that increase the risk of injury. Encouraging her to engage in regular physical activity, focusing on exercises that strengthen the shoulder and improve flexibility, can be beneficial.
Lastly, if your mother is experiencing frequent bruising without a clear cause, it may be worth discussing with her healthcare provider. They can evaluate her overall health, review any medications she is taking, and perform blood tests if necessary to rule out any underlying conditions that could be contributing to her symptoms.
In summary, shoulder pain and bruising after light activity can result from muscle strain, microtrauma to blood vessels, or more serious underlying conditions. Monitoring her symptoms and seeking medical advice if they persist or worsen is essential for proper diagnosis and treatment. Encouraging a balanced lifestyle with regular exercise and proper body mechanics can also help prevent future issues.
