Pain near the wrist and thumb?

Hello Doctor: For the past two months, I have been experiencing pain from my left thumb to my wrist (sometimes extending a bit further up, about one-third of my forearm) when using strength or performing twisting motions.
For example, after washing a teacup, when inverting the teacup, unlocking a padlock, or lifting heavy objects, etc.
I also feel pain when pressing on the area near my wrist.
What could this symptom indicate? It seems that heat therapy has not provided significant relief.
Should I seek medical attention? Or is it possible that medical treatment may not provide a complete cure? Please assist me with your response.
Thank you.

Dr. Jian Jincheng reply Rehabilitation

Pain in the left thumb and discomfort after washing teacups may indicate left thumb tendonitis.
Medical treatment is necessary.
Rehabilitation can be beneficial.

The symptoms you are experiencing in your left wrist and thumb area could be indicative of several conditions, most commonly related to overuse or strain injuries. Given that the pain occurs during specific movements, such as twisting or lifting, it suggests that you may be dealing with a repetitive strain injury, which is often seen in individuals who perform tasks that require repetitive wrist and hand motions.

One of the most common conditions that could be causing your symptoms is De Quervain's tenosynovitis. This condition involves inflammation of the tendons that run along the thumb side of the wrist, particularly affecting the abductor pollicis longus and extensor pollicis brevis tendons. Symptoms typically include pain and tenderness near the base of the thumb, which may radiate up the forearm, especially during activities that involve gripping or twisting motions. You might also notice swelling in the area and a "catching" sensation when moving your thumb.

Another possibility is wrist tendonitis, which can occur due to repetitive motions or overuse, leading to inflammation of the tendons in the wrist. This can cause pain during movement and tenderness upon palpation. Conditions like carpal tunnel syndrome could also be considered, especially if you experience numbness or tingling in the thumb or other fingers, although your description does not specifically mention these symptoms.

Since you have mentioned that heat therapy has not provided significant relief, it may be beneficial to explore other treatment options. Resting the affected area is crucial; avoiding activities that exacerbate the pain can help reduce inflammation. Additionally, using a wrist brace or splint may provide support and limit movement, allowing the tendons to heal.

If symptoms persist despite conservative measures, it would be advisable to seek medical attention. A healthcare provider can perform a physical examination and may recommend imaging studies, such as an X-ray or MRI, to rule out other potential issues, such as fractures or more severe tendon injuries. They may also refer you to a physical therapist who can provide targeted exercises to strengthen the wrist and improve flexibility, which can be beneficial in recovery.

In some cases, if conservative treatments fail, corticosteroid injections may be considered to reduce inflammation and pain. Surgery is typically a last resort and is only considered if other treatments do not alleviate the symptoms.

In summary, the pain you are experiencing in your wrist and thumb is likely related to a repetitive strain injury, such as De Quervain's tenosynovitis or wrist tendonitis. It is essential to rest the affected area, consider using a brace, and consult with a healthcare professional if symptoms do not improve. Early intervention can help prevent the condition from worsening and promote a quicker recovery.
