Right shoulder tightness and weakness in the entire right arm?

Hello: Several years ago, I had a minor strain in my right shoulder, but I did not experience any discomfort until the end of last year when I developed severe pain in my right shoulder.
Initially, it was just pain and discomfort in the right shoulder, but it gradually progressed to pain and weakness in my entire right arm, and my left shoulder also started to hurt due to the pain in my right shoulder.
At one point, I experienced pain throughout my back and lower spine.
Earlier this year, I consulted a doctor at a well-known hospital branch in Changhua, but the doctors kept referring me to other specialists, and after 2-3 referrals, they still could not identify the cause, despite me spending over a thousand dollars at that hospital.
I had no choice but to seek treatment from a traditional Chinese medicine practitioner, which provided some slight improvement, but I still experience intermittent pain in my right shoulder.
Given this situation, should I continue with traditional Chinese medicine treatment, or should I seek treatment at another hospital? Additionally, how can I manage this pain on my own for better self-care?

Hello: There are many causes of right shoulder pain.
If the pain radiates down the entire arm causing soreness and weakness, it is important to consider the possibility of cervical radiculopathy (due to herniated disc compression).
It is advisable to seek further evaluation from a rehabilitation specialist (such as X-rays or MRI, etc.).
Thank you for your inquiry.

Hello,
It sounds like you are experiencing significant discomfort and pain in your right shoulder, which has been ongoing for quite some time. Given your history of a previous minor injury and the recent escalation of symptoms, it’s understandable that you are seeking clarity on how to manage this condition effectively.

From your description, it seems you may be dealing with a combination of issues, possibly including rotator cuff injury, shoulder impingement, or even referred pain from cervical spine problems. The fact that your pain has progressed to involve your entire arm and even your left shoulder suggests that there may be underlying issues that need to be addressed.

Treatment Options
1. Medical Evaluation: Since you have already seen multiple doctors without a definitive diagnosis, it might be beneficial to seek a second opinion from an orthopedic specialist who focuses on shoulder injuries. They may recommend imaging studies such as an MRI to assess the soft tissues around the shoulder, including the rotator cuff and bursa.

2. Physical Therapy: Engaging in a structured physical therapy program can be highly beneficial. A physical therapist can provide you with exercises tailored to strengthen the shoulder muscles, improve flexibility, and reduce pain. They can also teach you proper body mechanics to avoid further injury.

3. Pain Management: Over-the-counter anti-inflammatory medications (like ibuprofen or naproxen) can help manage pain and reduce inflammation. If the pain is severe, your doctor may prescribe stronger medications or corticosteroid injections to provide relief.

4. Alternative Therapies: Since you mentioned that you have tried traditional Chinese medicine and found some relief, you might consider continuing with that approach if it helps. Acupuncture, massage therapy, and other modalities can be effective for some individuals in managing chronic pain.

5. Activity Modification: It’s essential to modify your activities to avoid exacerbating the pain. Avoid heavy lifting and overhead activities until you have a clearer understanding of your condition. Gentle stretching and strengthening exercises can be beneficial, but they should be done cautiously.

Self-Care Strategies
1. Heat and Cold Therapy: Applying heat can help relax and loosen tissues and stimulate blood flow to the area. Cold therapy can reduce inflammation and numb sharp pain. You can alternate between the two based on what feels best for you.

2. Gentle Stretching: Incorporate gentle stretching exercises for your shoulder and neck. This can help maintain mobility and prevent stiffness. However, avoid any movements that cause pain.

3. Posture Awareness: Pay attention to your posture, especially if you spend long hours sitting or working at a desk. Poor posture can contribute to shoulder and neck pain.

4. Ergonomic Adjustments: If your work involves repetitive motions or prolonged sitting, consider making ergonomic adjustments to your workspace to reduce strain on your shoulder.

5. Mindfulness and Relaxation Techniques: Stress can exacerbate pain perception. Techniques such as deep breathing, meditation, or yoga can help manage stress and improve your overall well-being.

Conclusion
In summary, while you may find some relief through alternative therapies, it’s crucial to pursue a comprehensive evaluation from a qualified orthopedic specialist to rule out any serious underlying conditions. A combination of medical treatment, physical therapy, and self-care strategies can significantly improve your shoulder pain and function over time. If your symptoms persist or worsen, do not hesitate to seek further medical attention. Your health and comfort are paramount, and finding the right treatment plan is essential for your recovery.

Best wishes for your healing journey!
