Premature closure of the growth plates?

Hello, doctor.
My son is currently 14 years and 4 months old.
Last week, he mentioned that his growth plates are closing quickly, and there is only a little space left at his wrist.
His current height is 172.7 cm (he has only grown 0.7 cm in the past three months).
It is estimated that he has at most another year and a half to grow, with a maximum potential growth of 3 to 4 cm, or possibly even less.
Both my husband and I have an average height of 175 cm.
Throughout his life, he has maintained a normal lifestyle (going to bed early, not being picky with food, drinking fresh milk daily, and skipping rope for 500 times).
The doctor has prescribed arginine, lysine, vitamin B6, and zinc for him.
I would like to know if it would be very difficult for him to reach a height of 178 cm through a combination of nutritional supplements and a proper lifestyle.
Thank you for your patient response, doctor!

Dr. Shi Guozheng reply Orthopedics

A balanced diet is sufficient, and there is no need to consume additional supplements as they may not provide significant benefits.
It is possible for a fourteen-year-old to grow taller.

To address your question about whether nutrition and lifestyle can help your son reach a height of 178 cm, we need to consider several factors that influence growth during adolescence. At 14 years and 4 months, your son is at a critical stage in his growth and development.
Firstly, it's important to understand that genetics plays a significant role in determining height. The average height of parents can provide a rough estimate of a child's potential height, but it is not the sole determinant. In your case, with an average parental height of 175 cm, your son is already close to this average at 172.7 cm.
The growth plates, or epiphyseal plates, are areas of developing cartilage tissue near the ends of long bones. Once these plates close, which typically occurs after puberty, further growth in height is not possible. You mentioned that your son’s growth plates are nearing closure, which indicates that he may have limited time left for growth. The fact that he has only grown 0.7 cm in the last three months suggests that his growth rate is slowing down, which is common as children approach the end of their growth spurts.

Now, regarding nutrition and lifestyle, these factors can indeed support optimal growth during the years when growth plates are still open. A balanced diet rich in essential nutrients is crucial. Key nutrients that promote growth include:
1. Protein: Essential for growth and development. Foods like lean meats, fish, eggs, dairy products, legumes, and nuts should be included in his diet.

2. Calcium: Important for bone health. Dairy products, leafy greens, and fortified foods are good sources.

3. Vitamin D: Helps the body absorb calcium. Sun exposure and foods like fatty fish, fortified milk, and egg yolks can provide vitamin D.

4. Zinc and Amino Acids: As you mentioned, supplements like arginine, lysine, and zinc can support growth, especially if dietary intake is insufficient. However, it's best to consult with a healthcare provider before starting any supplementation.

5. Overall Caloric Intake: Ensuring that your son consumes enough calories to support his growth is essential. A caloric deficit can hinder growth, even if nutrient intake is adequate.

In addition to nutrition, lifestyle factors such as regular physical activity, adequate sleep, and good hydration are vital. Your son’s routine of early sleep and regular exercise, such as jumping rope, is beneficial. Exercise stimulates growth hormone production, which can aid in growth during the developmental years.

However, it’s important to set realistic expectations. While a combination of proper nutrition, exercise, and sleep can optimize growth potential, reaching a specific height like 178 cm may be challenging given the current growth rate and the nearing closure of growth plates.
In conclusion, while nutrition and lifestyle can support your son’s growth, the potential for him to reach 178 cm may be limited. It would be wise to continue monitoring his growth and consult with a pediatrician or an endocrinologist who specializes in growth and development for personalized advice and to discuss any concerns regarding his growth trajectory. They can provide guidance on whether the supplements prescribed are appropriate and if any additional interventions may be beneficial.
