Inability to bend the toes after a car accident?
Hello, doctor.
It has been six months since my car accident, and the injured area is my left foot's big toe, which has some hairline fractures.
It is somewhat better than before, but it still feels a bit stiff when bending.
It doesn't move as smoothly as my adjacent big toe.
Although the bones are fine, I am unable to bend it properly.
The X-ray showed no issues according to the doctor.
Which specialty should I see for this, rehabilitation medicine or neurology?

Dr. Wang Zhaomin reply Rehabilitation
After a toe fracture from a car accident, there may be complications such as joint adhesion or contracture, leading to limited mobility and potential pain that can affect walking.
It is recommended to visit a rehabilitation specialist for evaluation and to arrange for rehabilitation therapy.

After a car accident, it's not uncommon to experience lingering issues, especially with injuries to the toes or fingers. In your case, you mentioned having a fracture in your left toe and experiencing stiffness and difficulty bending it, even after six months. Given your symptoms and the nature of your injury, it’s essential to approach the right specialist for further evaluation and treatment.
1. Orthopedic Specialist: Since you have a history of a toe fracture, the first specialist you should consider consulting is an orthopedic doctor. Orthopedic specialists focus on the musculoskeletal system, which includes bones, joints, ligaments, tendons, and muscles. They can assess the healing process of your toe and determine if there are any underlying issues that may be causing the stiffness. They may also recommend imaging studies, such as X-rays or MRIs, to evaluate the condition of the bone and surrounding tissues.
2. Physical Medicine and Rehabilitation (PM&R) Specialist: If the orthopedic evaluation indicates that the fracture has healed properly but you are still experiencing stiffness, a PM&R specialist, also known as a rehabilitation doctor, can be very helpful. They focus on restoring function and improving quality of life for patients with musculoskeletal injuries. They can create a tailored rehabilitation program that includes physical therapy, exercises, and other modalities to improve your range of motion and reduce stiffness.
3. Physical Therapist: Regardless of whether you see an orthopedic specialist or a PM&R doctor, working with a physical therapist is often beneficial. Physical therapists are trained to help patients regain strength and mobility after injuries. They can provide specific exercises to improve flexibility and strength in your toe and foot, which may alleviate the stiffness you are experiencing.
4. Neurologist: If there are concerns about nerve involvement or if you experience symptoms such as numbness or tingling in addition to the stiffness, consulting a neurologist may be warranted. However, based on your description, it seems that your primary concern is related to the musculoskeletal aspect of your injury, making an orthopedic or PM&R specialist more appropriate at this stage.
In summary, start with an orthopedic specialist to evaluate the healing of your toe fracture. If they determine that the bone has healed correctly but you still have stiffness, they may refer you to a PM&R specialist or recommend physical therapy. It’s important to address these issues promptly, as prolonged stiffness can lead to decreased function and potential complications in the long run.
Additionally, ensure that you communicate all your symptoms clearly during your appointments. This includes any pain, stiffness, or difficulty with movement. The more information you provide, the better your healthcare provider can tailor a treatment plan to your specific needs. Remember, recovery can take time, and following a structured rehabilitation program can significantly enhance your recovery process.
