Pain when raising the arm?
Hello, doctor.
I fell off my motorcycle and landed on my right shoulder.
Now, whenever I lift my arm above a certain height, I experience significant pain in the shoulder area.
I have seen an orthopedic doctor several times, and they diagnosed that there are no issues with the bones, advising me to rest for three months and return for a follow-up.
It has been two months, and the pain has not improved at all.
Should I consider seeing a rehabilitation specialist for further evaluation, or is there something specific I should be concerned about? I'm worried that if I wait longer, the situation might worsen.
Thank you for your help.

Dr. Wang Huijun reply Rehabilitation
Hello, based on the situation you described, it seems that your right shoulder has been injured.
Although there are no issues with the bones, the ligaments in your shoulder have been damaged.
It is important to start rehabilitation as soon as possible; otherwise, delaying it may often lead to the development of adhesive capsulitis (frozen shoulder).
Therefore, your thinking is correct.
The persistent pain indicates that inflammation is still present, so I recommend starting rehabilitation promptly.

More Info
Persistent shoulder pain after a motorcycle accident can be quite concerning, especially when it affects your ability to raise your arm without discomfort. Based on the information you've provided, it seems that you have already consulted with an orthopedic specialist who has ruled out any fractures or significant bone injuries. However, the ongoing pain and limited range of motion suggest that there may be underlying soft tissue injuries that have not yet been addressed.
In cases like yours, it is common for individuals to experience injuries to the rotator cuff, which is a group of muscles and tendons that stabilize the shoulder joint and allow for a wide range of arm movements. Symptoms of a rotator cuff injury can include pain when lifting the arm, weakness in the shoulder, and difficulty performing overhead activities. Given that your pain persists even after two months, it may be indicative of a rotator cuff tear or tendinitis, which can occur due to trauma or overuse.
Since your orthopedic doctor has recommended a period of rest, it is essential to follow that advice, but if you are not seeing any improvement, it may be time to consider further evaluation. Physical therapy can be highly beneficial in these situations. A physical therapist can assess your shoulder's range of motion, strength, and functional capabilities. They can also provide targeted exercises to help improve your shoulder's function and reduce pain. Additionally, they may employ modalities such as ultrasound therapy, electrical stimulation, or manual therapy techniques to alleviate pain and promote healing.
If you decide to switch to a rehabilitation specialist, they can perform a more comprehensive evaluation, which may include imaging studies like an MRI to assess the soft tissues around the shoulder. This can help identify any tears in the rotator cuff or other soft tissue injuries that may not have been visible on initial X-rays.
It's also important to monitor your symptoms closely. If you experience increased pain, swelling, or any new symptoms such as numbness or tingling in your arm or hand, you should seek medical attention promptly. These could be signs of more serious issues, such as nerve impingement or other complications.
In summary, while it is understandable to feel anxious about your ongoing pain, there are steps you can take to address the issue. Transitioning to a rehabilitation specialist for further evaluation and treatment may be beneficial. They can provide you with a tailored rehabilitation program to help restore your shoulder's function and alleviate pain. Remember to communicate openly with your healthcare providers about your concerns and progress, as this will help them guide you toward the best course of action for your recovery.
