Your Health Issues: A Guide for College Students - Family Medicine

Recently, I have been experiencing several health issues?

Hello, Doctor Ye! Thank you for your response.
I am currently 21 years old and a university student.
Let me start from my childhood: when I was about 10 years old, I experienced palpitations and other issues, and a brain examination revealed abnormal electrical activity in my brain.
Since entering junior high school, my sleep has not been good; I often need to lie down for over an hour before I can fall asleep, and my sleep is very light.
I wake up after just a few hours or sometimes can't sleep at all.
Additionally, I dream a lot, almost every day.
The situation was similar in high school, but it became more severe, especially on Sunday nights before Monday classes, when I could hardly sleep.
As a result, my condition on Mondays is usually very poor.
Now that I am in university, I find myself unable to sleep due to overthinking, especially before exams.
A few months ago, I had stomach discomfort and was diagnosed with mild gastritis after an endoscopy.
Since August, I have been experiencing abdominal discomfort, including abdominal pain, and my bowel movements have been irregular, sometimes diarrhea and sometimes pellet-like.
Furthermore, I occasionally experience severe tinnitus, which is quite noticeable when I sleep.
I also tend to get easily anxious and put pressure on myself.
There was a period when I felt very down and gloomy, but after some time, it improved.
I would like to ask the doctor what might be the cause of these issues and which specialty I should consult.
Thank you!

Lin, 20~29 year old female. Ask Date: 2018/08/29

Dr. Ye Qianyu reply Family Medicine

Hello, based on your description, I recommend that you first inquire whether the childhood palpitations and abnormal brain waves require follow-up treatment.
Sleep issues are clearly your biggest concern at the moment, so I suggest you seek treatment from a psychiatrist.
Thank you for your question, and I wish you a speedy recovery!

Hello! Thank you for reaching out and sharing your experiences. It sounds like you have been dealing with a complex array of health issues that have significantly impacted your quality of life. As a college student, navigating these challenges can be particularly overwhelming, especially with the added pressures of academic performance and social interactions.

From what you've described, it seems you are experiencing a combination of physical and psychological symptoms. Your history of sleep disturbances, anxiety, gastrointestinal issues, and sensitivity to stress could be interconnected. Let's break down some of these concerns and explore potential avenues for support.

1. Sleep Issues: Difficulty falling asleep, shallow sleep, and frequent awakenings can be symptoms of anxiety or stress. The fact that you often find it hard to sleep before exams suggests that performance anxiety might be a contributing factor. Sleep is crucial for cognitive function and emotional regulation, so addressing this issue is essential. Techniques such as cognitive-behavioral therapy for insomnia (CBT-I), mindfulness meditation, and establishing a consistent sleep routine can be beneficial.

2. Gastrointestinal Symptoms: Your history of gastric inflammation and current abdominal discomfort could be linked to stress and anxiety. The gut-brain connection is well-documented; stress can exacerbate gastrointestinal issues, leading to symptoms like abdominal pain and changes in bowel habits. It’s important to consult a gastroenterologist to rule out any underlying conditions and discuss dietary changes that may help alleviate your symptoms.

3. Anxiety and Emotional Health: Given your experiences with anxiety, including feelings of nervousness and pressure, it may be helpful to seek support from a mental health professional. A psychologist or counselor can provide you with coping strategies to manage anxiety and help you process your feelings. Cognitive-behavioral therapy (CBT) is particularly effective for anxiety disorders and can help you develop healthier thought patterns.

4. Physical Symptoms: The heart palpitations and ear ringing (tinnitus) you mentioned could be related to anxiety or stress. It’s crucial to discuss these symptoms with a healthcare provider, as they may require further evaluation. A cardiologist can assess your heart health, while an audiologist can evaluate your hearing and ear-related symptoms.

5. Seeking Help: You mentioned wanting to know which specialist to see. Given your symptoms, starting with a primary care physician is a good idea. They can perform an initial assessment and refer you to specialists such as a psychiatrist for mental health concerns, a gastroenterologist for your digestive issues, or a sleep specialist if necessary.

6. Self-Care Strategies: In addition to professional help, consider incorporating self-care practices into your routine. Regular physical activity, a balanced diet, and mindfulness practices can significantly improve your overall well-being. Journaling your thoughts and feelings can also provide an outlet for stress and anxiety.

7. Support Systems: Don’t hesitate to lean on your support network, whether that’s friends, family, or campus resources. Many universities offer counseling services, which can be a valuable resource for students facing mental health challenges.

In summary, your symptoms are multifaceted and likely require a comprehensive approach to treatment. By addressing both the physical and psychological aspects of your health, you can work towards improving your overall well-being. Remember, seeking help is a sign of strength, and you deserve to feel better. Please take care of yourself, and I wish you the best on your journey to better health.
