Why can't I gain weight?

Hello, doctor! My height is 182 cm and my weight is 66 kg.
Although my BMI is within the normal range, I actually feel very thin.
My upper body looks similar to that of a refugee from Africa, and being tall makes my body proportions look quite unbalanced.
I consider myself to be a person who can eat a lot, but my gastrointestinal health is quite poor.
After eating, as soon as I feel full, I have the urge to have a bowel movement.
I've heard that a normal person typically has bowel movements about 1 to 2 times a day.
Could this be the main reason why I can't gain weight? What should I do? Please, doctor, I really need your help! Thank you!

Jidan Mian, 20~29 year old female. Ask Date: 2005/08/12

Dr. Shi Guimei reply Nutrition

Hello! Poor gastrointestinal function can indeed affect digestion and absorption, leading to difficulty in gaining weight.
I recommend visiting the "Food Information Network" of the Department of Health, selecting "Nutrition and Health" → "Balanced Nutrition" → "Recommended Dietary Allowances for Adults," and following the suggested amounts for various food groups.
Additionally, incorporate a moderate amount of physical activity each day.
If your weight still does not increase and you experience excessive bowel movements, it is advisable to seek medical attention.
Sincerely, Dietitian Shih Kuei-Mei, Director of the Nutrition Department, Taoyuan Hospital, Ministry of Health and Welfare.

Gaining weight can be a challenging endeavor for some individuals, especially when they have underlying digestive issues or a fast metabolism. Based on your height of 182 cm and weight of 66 kg, your BMI is within the normal range, but you feel that you are too thin and have concerns about your body proportions. It's important to understand that weight gain, particularly in a healthy manner, requires a combination of dietary adjustments, exercise, and possibly medical evaluation.

Understanding Your Digestive Issues
From your description, it seems that you experience a strong urge to defecate shortly after eating, which could indicate a few potential issues. The normal frequency of bowel movements can vary widely among individuals, but typically, having 1-2 bowel movements per day is considered normal. If you are experiencing more frequent urges to defecate, it could be a sign of a gastrointestinal issue such as:
1. Irritable Bowel Syndrome (IBS): This condition can cause symptoms like bloating, gas, diarrhea, and constipation. It may lead to a rapid transit time, meaning food moves through your digestive system too quickly, preventing proper nutrient absorption.

2. Food Intolerances: Certain foods may not be well-tolerated by your digestive system, leading to discomfort and increased bowel movements. Common intolerances include lactose (found in dairy) and gluten (found in wheat products).

3. Malabsorption Syndromes: Conditions like celiac disease or pancreatic insufficiency can impair your body's ability to absorb nutrients from food, leading to weight loss or difficulty gaining weight.

Nutritional Strategies for Weight Gain
To gain weight healthily, consider the following strategies:
1. Increase Caloric Intake: Focus on consuming more calories than you expend. This can be achieved by incorporating calorie-dense foods into your diet. Foods such as nuts, seeds, avocados, whole grains, and healthy oils (like olive oil) can help increase your caloric intake without requiring you to eat large volumes of food.

2. Frequent Meals: Instead of three large meals, try eating five to six smaller meals throughout the day. This can help manage your digestive discomfort while ensuring you are consuming enough calories.

3. Protein-Rich Foods: Incorporate protein-rich foods into each meal, such as lean meats, fish, eggs, dairy products, legumes, and protein shakes. Protein is essential for muscle growth and repair, which is particularly important if you are looking to gain weight in a healthy manner.

4. Healthy Snacks: Keep healthy snacks on hand, such as yogurt, smoothies, protein bars, or trail mix. These can be consumed between meals to help increase your overall caloric intake.

5. Monitor Your Digestive Health: If your bowel habits continue to be a concern, it may be beneficial to consult with a healthcare professional or a gastroenterologist. They can help identify any underlying issues and recommend appropriate dietary changes or treatments.

Lifestyle Considerations
In addition to dietary changes, consider the following lifestyle adjustments:
- Exercise: Engage in strength training exercises to build muscle mass. Muscle weighs more than fat and can help improve your overall body composition. Focus on compound movements like squats, deadlifts, and bench presses.

- Stress Management: High stress can impact digestion and appetite. Incorporate stress-reducing activities into your routine, such as yoga, meditation, or deep-breathing exercises.

- Stay Hydrated: While hydration is essential, be mindful of drinking too much water before meals, as it may reduce your appetite. Instead, drink fluids between meals.

Conclusion
Gaining weight is a process that requires patience and consistency. By focusing on nutrient-dense foods, adjusting your meal frequency, and addressing any digestive issues, you can work towards achieving a healthier weight. If you continue to struggle with weight gain despite these efforts, seeking guidance from a registered dietitian or healthcare provider can provide personalized support and recommendations tailored to your specific needs.
