Methods to Improve Insulin Resistance

Hello, nutritionist.
I would like to inquire about dietary issues related to insulin resistance.
Here are the details: In March of last year, I discovered I had insulin resistance during a health check-up (pre-meal blood glucose 94 mg/dL; hemoglobin A1c 5.4%; pre-meal insulin 8.9 μIU/mL (normal range 5-15); insulin resistance index 2.1).
Prior to that, I frequently consumed sugary beverages, so after the examination, I primarily adjusted my diet by eliminating sugary drinks and paying attention to the sugar content in foods.
Recently, I went to a medical laboratory for a follow-up test, but my insulin resistance index has increased, which has made me feel a bit discouraged.
I'm unsure how else I can improve this; should I further restrict my carbohydrate intake? I will be going to work soon and would like to know how to choose better options when eating out (pre-meal blood glucose 94 mg/dL; hemoglobin A1c 5.3%; pre-meal insulin 18.9 μIU/mL (normal range 2.6-24.9); insulin resistance index 4.3).
Additionally, since the two tests were conducted at different locations, the reference ranges for "pre-meal insulin" on the reports are not the same.
The medical laboratory explained that this is due to different machines, and the earlier value can be roughly considered as half of the latter.
I'm unsure how this difference affects the insulin resistance index and whether it means my insulin resistance index has truly doubled.
I'm feeling quite discouraged and worried.
Just to add, my BMI is 18.1, and my lipid levels are all normal.

Hello, regarding insulin resistance, it may be related to being overweight, obesity, or endocrine system issues.
Currently, the glycated hemoglobin you mentioned is within the normal range, and your weight is slightly on the lower side, with no obesity concerns.
For the insulin issue, it would be advisable to consult an endocrinologist or a metabolic specialist for evaluation.
When eating out, it is important to pay attention to a balanced diet and reduce fat intake.
It is necessary to assess individual dietary habits and the factors influencing them to create a personalized dietary plan, as each person's situation is different and requires a detailed understanding.
This response is provided by the Nantou Hospital of the Ministry of Health and Welfare.
Wishing you peace and health.

More Info

Improving insulin resistance through dietary strategies is a crucial aspect of managing metabolic health, especially for individuals who have been diagnosed with insulin resistance. Based on the information you provided, it seems you have already made significant dietary changes, such as eliminating sugary beverages and being mindful of sugar intake. However, your recent lab results indicate a rise in your insulin resistance index, which understandably has caused you concern.

Understanding Insulin Resistance
Insulin resistance occurs when the body's cells become less responsive to insulin, a hormone that helps regulate blood sugar levels. This can lead to higher insulin levels in the blood as the pancreas works harder to compensate. Your initial blood tests showed a relatively normal fasting blood sugar and hemoglobin A1c, but the increase in your insulin resistance index suggests that your body is struggling to manage insulin effectively.

Dietary Strategies to Improve Insulin Resistance
1. Focus on Low Glycemic Index Foods: Foods that have a low glycemic index (GI) cause a slower rise in blood sugar levels. Incorporating whole grains, legumes, nuts, seeds, fruits, and non-starchy vegetables can help manage blood sugar levels more effectively. Avoiding high-GI foods, such as white bread, sugary snacks, and sweetened beverages, is essential.

2. Increase Fiber Intake: A diet high in fiber can improve insulin sensitivity. Aim for a variety of fiber sources, including fruits, vegetables, whole grains, and legumes. Fiber helps slow down the absorption of sugar and can lead to better blood sugar control.

3. Healthy Fats: Incorporate sources of healthy fats, such as avocados, olive oil, nuts, and fatty fish. These fats can help improve insulin sensitivity and provide satiety, which may help in managing overall caloric intake.

4. Regular Meal Timing: Eating at regular intervals can help maintain stable blood sugar levels. Consider smaller, more frequent meals rather than large meals, which can lead to spikes in blood sugar.

5. Limit Processed Foods: Processed foods often contain added sugars and unhealthy fats that can exacerbate insulin resistance. Focus on whole, unprocessed foods as much as possible.

6. Portion Control: Even healthy foods can contribute to weight gain if consumed in large quantities. Be mindful of portion sizes, especially with carbohydrate-rich foods.

7. Hydration: Staying hydrated is essential. Water is the best choice, and avoiding sugary drinks can significantly reduce your overall sugar intake.

Eating Out Strategies
When dining out, consider the following tips:
- Choose Grilled or Baked Options: Opt for grilled, baked, or steamed dishes instead of fried foods.

- Ask for Dressings and Sauces on the Side: This allows you to control the amount you consume.

- Select Whole Grains: If available, choose whole grain options for bread or rice.

- Load Up on Vegetables: Aim to fill half your plate with non-starchy vegetables.

- Be Mindful of Portions: Restaurant portions can be large; consider sharing a dish or asking for a half portion.

Understanding Your Lab Results
Regarding the differences in lab results, it’s important to note that variations in testing methods and equipment can lead to discrepancies in results. If the reference ranges differ significantly, it may not necessarily indicate a true doubling of your insulin resistance. It’s advisable to discuss these results with your healthcare provider, who can interpret them in the context of your overall health and provide personalized recommendations.

Conclusion
Improving insulin resistance is a multifaceted approach that includes dietary changes, regular physical activity, and possibly medical intervention if necessary. Given your current BMI of 18.1, which is on the lower end, it’s crucial to ensure that any dietary changes do not lead to unintended weight loss or nutritional deficiencies. Consulting with a registered dietitian or a healthcare provider can provide tailored guidance to help you effectively manage your insulin resistance while maintaining overall health.
