Left-sided upper back pain?

Hello doctor, two days ago I woke up with sudden pain in my left shoulder, which extended to the back of my left side.
After resting in bed for about five minutes, the pain lessened, but I noticed that certain movements, like hiccuping and yawning (which cause more pain), as well as turning over, tend to trigger the pain.
Could this be a back strain or something else? Is it okay to apply heat? Thank you.

More Info

Left-sided upper back pain can be a common issue, often arising from various causes, including muscle strain, poor posture, or even underlying medical conditions. Given your description of the pain starting suddenly upon waking and being exacerbated by certain movements like yawning or turning, it is likely that you are experiencing a muscular issue, possibly a strain or spasm in the muscles of the upper back or shoulder area.

Possible Causes
1. Muscle Strain: This is one of the most common causes of sudden back pain. It can occur due to awkward sleeping positions, sudden movements, or overuse of the muscles. If you sleep on your side, the weight of your body can put pressure on your shoulder and upper back, leading to muscle strain.

2. Poor Posture: If you have a habit of slouching or sitting in a way that puts stress on your back, this can lead to muscle fatigue and pain. Poor posture during sleep can also contribute to discomfort.

3. Nerve Compression: Sometimes, a herniated disc or other spinal issues can compress nerves that lead to pain in the upper back. This is less likely if the pain is localized and does not radiate down the arm.

4. Referred Pain: Pain can sometimes be referred from other areas, such as the neck or even the heart. However, given your description, it seems more likely to be muscular in nature.

5. Underlying Conditions: Conditions such as fibromyalgia, arthritis, or even infections can cause upper back pain, but these are usually accompanied by other symptoms.

Remedies and Management
1. Rest: Allow your body some time to heal. Avoid activities that exacerbate the pain.

2. Heat Therapy: Applying heat to the affected area can help relax the muscles and alleviate pain. A warm compress or heating pad can be beneficial. However, if there is swelling, ice may be more appropriate initially.

3. Gentle Stretching: Once the acute pain subsides, gentle stretching and mobility exercises can help restore flexibility and strength to the muscles. Focus on stretches that target the upper back and shoulders.

4. Posture Correction: Pay attention to your posture during daily activities and while sleeping. Consider using supportive pillows that keep your spine aligned.

5. Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help reduce pain and inflammation.

6. Physical Therapy: If the pain persists, consulting a physical therapist can provide you with tailored exercises and treatments to strengthen the muscles and improve posture.

7. Consult a Doctor: If the pain does not improve with home care, or if you experience additional symptoms such as numbness, tingling, or weakness in the arms, it is important to seek medical attention. A healthcare provider can perform a physical examination and may recommend imaging studies like X-rays or MRI if necessary.

Conclusion
In summary, your left-sided upper back pain is likely due to muscle strain or poor posture, especially considering its onset after sleeping. Applying heat, resting, and practicing good posture can help alleviate your symptoms. If the pain persists or worsens, do not hesitate to consult a healthcare professional for further evaluation and treatment. Remember, early intervention can prevent more serious issues down the line.
