Nutritional supplements
Hello, nutritionist.
I would like to ask about a patient who has difficulty swallowing after cervical spine surgery.
If the patient is obese and has recovered post-surgery, would you still recommend supplements? The patient is an elderly individual with poor bone density.
If supplements are advised, would it be appropriate to consume a high-calcium formula (low sugar) supplement? Thank you.
Xiao Yu, 20~29 year old female. Ask Date: 2020/08/05
Dr. Cai Xiuwen reply Nutrition
Hello, if swallowing difficulties arise from cervical spine surgery and occur only during the preoperative and postoperative recovery period, normal eating can resume once the swallowing difficulties have resolved.
A more important consideration is whether the usual dietary habits are balanced and whether certain eating and lifestyle habits may lead to obesity.
Therefore, it is not recommended to supplement with nutritional products after recovery.
If the patient is an elderly individual with poor bone quality, it is essential to investigate the underlying causes.
In addition to insufficient calcium, other nutritional issues should be considered, such as overall caloric intake, protein sufficiency, and dietary habits that may adversely affect bone quality.
If there is an overall caloric deficiency or inadequate protein intake, simply supplementing with calcium will not improve bone quality.
This response is provided by Dietitian Tsai Hsiu-Wen from Nantou Hospital, Ministry of Health and Welfare.
Wishing you peace and health.

Post-surgery nutrition is crucial, especially for elderly patients who may face complications such as swallowing difficulties. In the case of an obese senior who has undergone cervical spine surgery and is experiencing issues with swallowing, the approach to nutrition and supplementation must be carefully considered.
First and foremost, it is essential to assess the patient's overall nutritional status. Obesity can often mask underlying nutritional deficiencies, and it is vital to ensure that the patient is receiving adequate nutrients to promote healing and recovery. After surgery, the body requires additional energy and nutrients to repair tissues, fight infections, and maintain overall health. Therefore, the focus should be on providing a balanced diet that meets the patient's caloric and nutritional needs.
If the patient has regained the ability to swallow but is still struggling with certain textures or consistencies of food, it may be beneficial to consider nutritional supplements. These supplements can provide concentrated sources of calories and nutrients, which can be particularly helpful for patients who may not be able to consume enough food through regular meals. However, it is crucial to choose the right type of supplement.
For an obese senior, it is advisable to select supplements that are lower in sugar and calories while still being rich in essential nutrients. High-calcium formulations can be beneficial, especially if the patient has poor bone density, as calcium is vital for bone health. However, it is important to ensure that the overall caloric intake does not contribute to further weight gain. Therefore, consulting with a registered dietitian or nutritionist can help tailor a supplementation plan that aligns with the patient's specific needs.
In addition to calcium, the patient should also be encouraged to consume adequate protein, as it plays a significant role in muscle repair and recovery. Protein-rich foods or supplements can help maintain muscle mass, which is particularly important for seniors who may be at risk of sarcopenia (age-related muscle loss).
Moreover, hydration is another critical aspect of post-surgery nutrition. Seniors often have a reduced sense of thirst, which can lead to dehydration. Ensuring that the patient is adequately hydrated can aid in recovery and overall health.
It is also essential to monitor the patient's weight and nutritional status regularly. If the patient is losing weight or not gaining weight as expected, adjustments to the diet or supplementation may be necessary.
In conclusion, while nutritional supplements can be beneficial for an obese senior recovering from surgery, it is essential to approach this with caution. The focus should be on providing a balanced diet that meets the patient's caloric and nutritional needs without contributing to further weight gain. High-calcium, low-sugar supplements can be considered, but they should be part of a comprehensive nutrition plan developed in consultation with healthcare professionals. Regular monitoring and adjustments based on the patient's progress will be key to ensuring optimal recovery and health.
