To ensure mother's health?

My mother is 63 years old, 158 cm tall, weighs 80 kg, and has a waist circumference of over 90 cm.
She has large and fatty thighs, abdominal obesity, and has been diagnosed with hypertension for which she has been taking diuretics prescribed by her doctor for several years.
Both of her legs have been injured in a car accident many years ago and she suffers from osteoarthritis and osteoporosis.
Due to her obesity and low physical activity, losing weight is very challenging for her.
I would like to ask Nutritionist Tsai for dietary recommendations that would be beneficial and practical for my mother.
Thank you! Grateful to Nutritionist Tsai!

Shu, 40~49 year old female. Ask Date: 2011/05/13

Hello! Based on your description, it is understood that due to currently low activity levels, central obesity is easily developed.
For hypertension and osteoporosis, in addition to dietary control, lifestyle changes play a significant role.
Therefore, it is recommended to adjust both diet and activity levels.
Relying solely on diet will not be very helpful.

According to your current height, the ideal weight is 54.9 kg, but since your current weight is 80 kg, it is significantly above the ideal weight.
The target weight adjustment is set at 61.2 kg, with a daily caloric intake recommendation of 1520 calories.
The daily intake should consist of staple foods (2.5 bowls of rice), protein sources (5 ounces of eggs, beans, fish, and meat), 6 teaspoons of fats, 300 grams of vegetables, and 2 servings of fruits (each serving about the size of a fist).

Additionally, it is important to pay attention to cooking methods to achieve the target portions, as simply knowing the target amounts can easily lead to overlooking excess calorie intake.
For hypertension, be cautious about consuming high-sodium foods such as pickled vegetables, canned bamboo shoots, and other processed foods, and monitor blood lipid levels to limit high-fat diets.
Regarding the inability to exercise, lifestyle changes can be made to adjust activity levels.
Although running may not be possible, you can still engage in more activities, such as marching in place, twisting your waist, and swinging your arms for 10 minutes daily.
After a month, you can gradually increase the duration and intensity.
Additionally, you can perform mat exercises or increase water-based activities.
It is advisable to consult with a healthcare facility and a nutritionist based on biochemical reports and dietary habits for a detailed discussion.
Keeping a food diary can help better understand dietary issues, allowing for more effective adjustments to resolve them.
Wishing you and your mother good health.

Managing weight and health issues, especially for older adults, requires a thoughtful approach that considers their unique health conditions and lifestyle. Your mother's situation, with her age, height, weight, and existing health issues such as hypertension, osteoarthritis, and osteoporosis, necessitates a tailored dietary plan that promotes weight loss while ensuring she receives adequate nutrition.

Understanding the Challenges
At 63 years old, with a height of 158 cm and a weight of 80 kg, your mother has a Body Mass Index (BMI) of approximately 32, which classifies her as obese. This can significantly increase her risk for various health issues, including cardiovascular diseases, diabetes, and joint problems, especially given her history of joint injuries and arthritis. Additionally, her high blood pressure and the use of diuretics indicate that her diet must be carefully managed to avoid exacerbating these conditions.

Dietary Recommendations
1. Balanced Diet: Encourage a diet rich in whole foods, including fruits, vegetables, whole grains, lean proteins, and healthy fats. This can help manage weight while providing essential nutrients. Aim for a variety of colors on her plate to ensure a broad spectrum of vitamins and minerals.

2. Portion Control: Given her weight and health issues, portion control is crucial. Using smaller plates can help her manage portion sizes without feeling deprived. Encourage her to listen to her body's hunger cues and eat slowly to recognize when she is full.

3. Limit Processed Foods: Processed foods often contain high levels of sodium, sugars, and unhealthy fats, which can contribute to weight gain and worsen hypertension. Encourage her to prepare meals at home using fresh ingredients whenever possible.

4. Increase Fiber Intake: Foods high in fiber, such as fruits, vegetables, legumes, and whole grains, can help her feel full longer and support digestive health. Fiber can also help in managing blood sugar levels and cholesterol.

5. Hydration: Staying hydrated is essential, especially since she is on diuretics. Encourage her to drink plenty of water throughout the day and limit sugary drinks and excessive caffeine.

6. Healthy Snacking: If she feels hungry between meals, suggest healthy snacks such as nuts, yogurt, or fresh fruit. These options can provide nutrients without excessive calories.

7. Limit Salt Intake: Since she has high blood pressure, reducing sodium intake is vital. Encourage her to use herbs and spices for flavoring instead of salt and to read labels carefully to avoid high-sodium foods.

8. Regular Meal Times: Establishing a routine can help regulate her metabolism and prevent overeating. Encourage her to have regular meals and avoid skipping meals, which can lead to overeating later.

Physical Activity
While your mother may have limitations due to her joint issues, incorporating gentle physical activity is essential. Activities such as walking, swimming, or chair exercises can help improve her mobility and overall health. It’s important to consult with her healthcare provider before starting any new exercise regimen to ensure it’s safe and appropriate for her condition.

Professional Guidance
Given your mother's complex health needs, it may be beneficial for her to work with a registered dietitian or nutritionist who specializes in geriatric care. They can provide personalized meal plans and ongoing support to help her achieve her weight loss goals safely.

Conclusion
In summary, managing weight and health issues for your mother involves a comprehensive approach that includes a balanced diet, portion control, hydration, and gentle physical activity. By making gradual changes and focusing on whole, nutrient-dense foods, she can improve her health and well-being. Encouragement and support from family can also play a significant role in her success.
